Please provide the following information to claim either a 300 Game or 800 Series IGBO Award. If the bowler completed an 800 and 300 award in same series, PLEASE SUBMIT TWO SEPARATE REQUESTS - one for each. Upon submit, your form information will be sent to our awards administration team for review and processing.

In accordance with the IGBO award program she/he is entitled to one 300 and one 800 award per fiscal year (July 1 to June 30). Additional awards may be purchased for multiple honor scores bowled during a fiscal year. Contact Rob Silliman for additional orders.
